Eligibility > Forms > Surveys


Income Surveys function as an alternative to free and reduced application forms. These are typically used by districts engaging in a special provision program. Rather than applying for free and reduced benefits, they submit household income data in compliance with state funding programs.

ALL tab

The ALL tab displays all surveys based on the selected filters.

  1. Ensure that the ALL tab is selected
  2. Select an Academic Year from the dropdown
  3. Select Benefits from the dropdown
  4. Click the Apply button
    Income Surveys display below based on the selections.

View Survey Details

  1. Click the Survey Details icon for the desired survey
    The Survey Details page displays.
  2. View each section of the survey
  3. Click the section(s) Edit icon to edit the information if necessary

The following buttons are available for each application:

  • PROCESS SURVEY Use this button to process the survey and return to the Survey Details page. A Process Survey slide deck will display, requiring you to enter a comment before completing this action.
  • NOTIFY – Use this button to send a notification letter to the user via email or print. A Send Notification Letter slide deck will display, requiring you to select the Email/PrintOptions.
  • MORE ACTIONS – Use this button to mark as a pending survey and to delete the survey.
    • MARK AS PENDING SURVEY – Use this option to mark the survey as pending. The Mark as Pending Survey slide deck will display, requiring you to enter a comment.
    • DELETE SURVEY – Use this button to delete the survey. The Delete Survey slide deck will display, requiring you to enter a comment.

Click the BACK button to return to the ALL tab on the Income Surveys page. A Close Survey pop-up message will appear before you can return.

View Survey History

  1. Click the Survey History icon for the desired survey
    The Survey History slide deck displays.
  2. Click a row in the table
    Information appears in the Changed Value(s) section.
    A key notating the Old and New changed value(s) is available.
  3. View the Survey History information
    Click the X to close the slide deck.

PENDING tab

The PENDING tab displays all surveys with a Pending Status.

  1. Select the PENDING tab atop the page
    Surveys with a Pending Status automatically display.

View Survey Details

  1. Click the Survey Details icon for the desired survey
    The Survey Details page displays.
  2. View each section of the survey
  3. Click the section(s) Edit icon to edit the information if necessary

The following buttons are available for each application:

  • PROCESS SURVEY Use this button to process the survey and return to the Survey Details page. A Process Survey slide deck will display, requiring you to enter a comment before completing this action.
  • MORE ACTIONS – Use this button to mark as a pending survey and to delete the survey.
    • MARK AS PENDING SURVEY – Use this option to mark the survey as pending. The Mark as Pending Survey slide deck will display, requiring you to enter a comment.
    • DELETE SURVEY – Use this button to delete the survey. The Delete Survey slide deck will display, requiring you to enter a comment.

Click the BACK button to return to the ALL tab on the Income Surveys page. A Close Survey pop-up message will appear before you can return. Select the PENDING tab again to proceed.

View Survey History

  1. Click the Survey History icon for the desired survey
    The Survey History slide deck displays.
  2. Click a row in the table
    Information appears in the Changed Value(s) section.
    A key notating the Old and New changed value(s) is available.
  3. View the Survey History information
    Click the X to close the slide deck.
